Abstract Introduction Ceramic on ceramic bearing in Total Hip Replacement (THR) is associated with a low but evident risk of linear fracture, which can lead to catastrophic failure if not diagnosed and treated early. Case presentation: A 59-year-old male patient, who underwent ceramic-on-ceramic THR 8 years ago, presented our centre five-week history right hip pain, after he suddenly twisted his upper body felt sharp pain groin. Prior the injury had an excellent functional level prosthesis. His examination findings confirmed reduced range movement groin internal external rotation.
